Geriatric Medicine department specializes in the comprehensive multi-disciplinary geriatric assessment and treatment of senior citizens.     Specialized Geriatric Services (SGS) is a network of specialty teams in ambulatory, outreach and inpatient settings that specialize in the assessment, intervention, and management of older persons with functional decline, frailty, and complexities. A Geriatrician­ directs multidisciplinary team and takes a holistic approach to patient care that involves clinical, psychosocial and functional assessment. All the patients who are referred to our service undergo full comprehensive geriatric assessment.

    Our team of experts address many concerns in areas such as nutrition, sexuality and functional independence. Our goal is to provide comprehensive services for today’s seniors to help them achieve and maintain a high quality of life as they age.

    Our Care Team

    The health care professionals look at the person holistically. The team considers the person’s medical history and present health conditions. The team also looks for the effects of past illnesses, as well as ‘geriatric syndromes’ – common health concerns found in frail elderly such as

    PARKINSON’S EDUCATION PROGRAMME

    This six week program increases awareness and provides coping strategies for individuals with Parkinson’s disease and their care partners

    What to expect during each visit

    One hour education session provided by skilled health care professionals to address the following areas:

    • Signs and symptoms of Parkinson’s disease
    • Disease progression and management of physical symptoms
    • Medication management
    • Mobility issues
    • Speech and swallowing issues and nutrition
    • Adaptive equipment for self care
    • Energy conservation and relaxation
    • Health benefits of leisure
    • Overcoming barriers to leisure
    • Social and emotional aspects of the disease and adjustment process
    • One hour exercise class, led by a physiotherapist

    Participants are provided with comprehensive literature from the various components of the program. The program is intended for individuals living with Parkinson’s disease and their care partners. Other eligibility criteria include:

    • Ability to participate in group activities
    • Independence with transfers and going to the toilet
    • Ability to participate in an exercise class is evaluated on an individual basis
