Day: 1

Day: 2-15
Patient meets the doctor and gets the evaluations and test list necessary for the procedure.

Other aspects of your health-like your heart, lungs, kidneys, immune system, and mental health-will also be checked to be sure you’re strong enough for surgery.
(During investigations, the recipient is checked for severity, cause and contraindications for liver transplant.)
Screening for Liver Transplant Donor:
Donor has to be a matching blood group within the second degree of relation to the patient. He will have extensive medical and psychological evaluations to ensure the lowest possible risk. Blood type and body size are critical factors in determining who is an appropriate donor. Following this, he is screened for a few important tests to determine compatibility with the recipients.
These are:
- Blood type
- Crossmatch
- HLA testing to prove relationship between recipient and the donor
- CT scan to assess the volume and fat
- MRI to assess bile duct (optional)
- Ideally the donor should be between 18-50 years of age and free from any comorbidities such as Hepatitis virus, Diabetes etc.
- ABO incompatible liver transplant is also an option for patients who have an unmatched donor.
Paper Preparation:
There are certain papers to be prepared before the transplant procedure to prove the donor relationship with the recipient and to get the approval form the authorization committee. Medical proof of relationship can be established through:
HLA typing:
- If HLA doesn’t match, a DNA test can also be conducted. DNA results are available within 7-10 days.
- Once legal committee clearance is received – both donor and recipient are admitted to the hospital.
Transplant Procedure:
- Prior to the surgery, additional blood tests, an electrocardiogram, and a chest X-ray will generally be taken.
- Donor surgery starts about 1 hour before the recipient’s surgery.
- In a live donor liver transplant, a portion of the liver is surgically removed from a live donor and transplanted into a recipient immediately after the recipient’s liver has been entirely removed.
- About 60-70% of the donor liver is retrieved and placed in the recipient body. Donor regrows the liver from leftover 30% to 95% within 3 months.
- Surgery for the donor lasts about 6 hours while for the recipient about 8 hours.
- The surgery for the donor can be done laparoscopically so hardly any visible scar. The surgery is very safe and the risk to the life of the donor is less than 0.1%.
After Procedure:
- Both donor and recipient will be shifted in the intensive care unit for very careful monitoring of all body functions, including the liver.
- Once the patient is transferred to the ward, the frequency of blood testing, etc. is decreased, eating is allowed and physiotherapy is prescribed to regain muscle strength.
- Initially the drugs will be given by vein to prevent rejection, but later by mouth.
- The Donor will be admitted in the hospital for 7 days with one day in the ICU.
- The recipient will stay 21 days including 4-5 days in the ICU.
Post discharge:
- The donor is advised to stay for 1 week post discharge near the hospital and then he can return to his home country.
- Donors can resume a normal work routine within 4 weeks of the surgery.
- However, donors are advised to avoid heavy weight lifting for about 3 months.
- While the Recipient is advised to stay for about 1 month near the hospital.
- The liver transplant patient needs to maintain good hygiene at the office and home.
- He needs to take lifelong immune-suppressions.
- With good personal care, doctors’ advice, regular medications, He can lead a normal life.
- Liver transplant changes the quality of life for recipients drastically.
- Recipients are advised to maintain discipline in terms of medicine regime etc.
- After 3 months, the recipient goes back to normal life.
- 80% of liver recipients live near normal life and have very good life expectancy.
